The objective of the Transition Phase is to migrate changes from the staging to the production environment. The milestone for this phase is to release features or changes to the Operations and Support team so they can make it available to the end user in the production environment. This phase requires special attention from the team if the product is already commercial. Migrations to the production environment must be planned and carrefully carried out so problems to the production site do not happen.

Deployment
Deployment:

The objective of this workflow is to deliver changes to the Operations and Support group so they can lead the configuration process in the operational environment. To do that you must ensure that the needs of the operations and support group are met — after all, these are the people that will keep the application running once it is in production. Each deployment is determined by small releases of a set of features. After a few iterations, features or changes that a part of the current release should be delivered to the operations and support team that will make it available to the user community. The Release Planning determines which set of features should be delivered in each release.
